ABSTRACT

Se describe el caso clínico de una paciente de 18 años de edad, quien acudió a la Clínica Estomatológica Provincial Docente Mártires del Moncada de Santiago de Cuba, por presentar persistencia de caninos temporales. Para el diagnóstico y tratamiento exitoso fue necesaria la participación de un equipo multidisciplinario integrado por especialistas en: ortodoncia, imagenología, prótesis y cirugía maxilofacial, quienes corroboraron la retención de caninos superiores. Mediante el tratamiento conjunto de ortodoncia y prótesis, se logró la rehabilitación estética y funcional de la paciente.


The case report of an 18 years patient is described. She went to Mártires del Moncada Teaching Provincial Stomatologic Clinic in Santiago de Cuba, due to persistence of temporary canine teeth. The participation of a multidisciplinary team integrated by specialists in: orthodontics, imaging, prosthesis and maxillofacial surgery was necessary for the successful diagnosis and treatment. They corroborated the retention of upper canine teeth. By means of the combined treatment of orthodontics and prosthesis, the patient cosmetic and functional rehabilitation was achieved.

ABSTRACT

OBJECTIVE@#To observe the effect of electroacupuncture (EA) on the rehabilitation of knee joint function after anterior cruciate ligament (ACL) reconstruction.@*METHODS@#A total of 140 patients with ACL reconstruction were randomly divided into an observation group (58 cases recruited, 12 cases dropped out) and a control group (65 cases recruited, 5 cases dropped out). The patients in the control group were treated with routine rehabilitation treatment. The patients in the observation group, on the basis of the treatment in the control group, were treated with EA at Fengshi (GB 31), Futu (ST 32), Zusanli (ST 36), Shangjuxu (ST 37), Fenglong (ST 40), Xuanzhong (GB 39), Diji (SP 8) and Sanyinjiao (SP 6) on the affected side (2 Hz/100 Hz of dilatational wave, 2-5 mA). Each EA treatment lasted 20-30 min, twice a day for 7 days. The swelling degree (d), pain visual analogue scale (VAS), knee joint range of motion (ROM), scores of International Knee Documentation Committee (IKDC) subjective short form and scores of Lysholm were observed in the two groups 1 day, 1 month, 3 months, 6 months and 1 year after operation.@*RESULTS@#One month and 3 months after operation, the swelling degree (d) and VAS scores in the observation group were lower than those in the control group (0.05). One month, 3 months, 6 months and 1 year after operation, the ROM of the knee joint in the observation group was higher than that in the control group (<0.05), the IKDC score and Lysholm score were higher than those in the control group (<0.05). Within one year, there were no relaxations, fractures and other related complications in the two groups. The pivot shift test, anterior drawer test and the Lachman test were all negative.@*CONCLUSION@#EA combined with routine rehabilitation training could obviously reduce the pain of knee joint, improve the swelling degree, increase the ROM of knee joint, promote the functional recovery in patients with ACL reconstruction, which are superior to rehabilitation training alone.

OBJECTIVE@#To observe the therapeutic effect of electro-acupuncture on tendon healing and functional recovery of rotator cuff injury in rats and explore the therapeutic mechanism of electro-acupuncture.@*METHODS@#Ninety SD rats were randomly divided into electro-acupuncture group, model group and blank control group, and models of rotator cuff injury were established in the former two groups.The rats in electro-acupuncture group was treated with electro-acupuncture after the operation, and those in the other two groups received no treatment.The right forefoot thermal withdrawal latency (TWL), the contents of IL-1β, IL-6 and TNF-α in the synovial fluid and the maximum tension load of supraspinatus tendon were measured at 2, 4 and 8 weeks after the operation.@*RESULTS@#TWL in the model group was significantly lower than that in the blank control group and electro-acupuncture group at 2, 4 and 8 weeks after the operation (@*CONCLUSIONS@#Electro-acupuncture treatment not only effectively reduces the expression of inflammatory factors to relieve pain, but also promotes the repair of damaged tissue to improve the biomechanical properties of rotator cuff in the rat models.

ABSTRACT

Objective:To analyze the categories and its relations of recreational physical activities and rehabilitation exercise for adult with disabilities based on the theory and method of International Classification of Functioning, Disability and Health (ICF). Methods:An evidence-based scoping review had been implemented. We had developed the framework and coding approaches for recreational physical activities, rehabilitation exercise and functional improvement for the adults with disabilities based on evidence-based literature review. The literature related to recreational physical activities and rehabilitation exercise of adult with disabilities was searched by subject retrieval method. The retrieval database in Chinese included CNKI and Wanfang database, and the retrieval database in English included PubMed and Web of Science. The retrieval time was from establishment to July 31, 2020. The authors, countries, published time, published journals, research objectives, object of the study and their ages, data collection tool, and the key findings were extracted. Results:A total of 2476 literatures were returned, and 18 of them were enrolled, which were published in 14 countries, and most were published in journals on topics related to medicine, public health, exercise and people with disabilities. They were mainly published from 2010 to 2020, and mainly used questionnaire survey, measurement and interviewing method. There were three typical recreational physical activities: physical fitness activities, skills activities and sports activities, mainly including Taiji Quan, wheelchair basketball, leisure outdoor activities, community recreational sports activities and rehabilitation exercises, etc. According to the framework of ICF, recreational physical activities promote functional rehabilitation for adults with disabilities, including physical functions (b1 mental function; b4 cardiovascular, blood, the function of the immune system and respiratory system; b5 digestion, metabolism and function of the endocrine system; and b7 nerve musculoskeletal function related to movement), and activities and participation (d1 study and application of knowledge; d2 the general tasks and requirements; d4 activity; d5 self-care; d7 interpersonal communication and interpersonal relationship; and d9 community, social and civic life). The evidences showed benefits of improvement in the fields of body mass index, body index, athletic ability, balance ability, and cognitive function. There were more gains at individual levels, including support, coping with stress, self-concept development, life satisfaction, happiness, exercise self-efficacy, health perception, etc. The environmental and personal factors had effects on the performance of activity and participation in recreational physical activities, including e1 products and technologies, e4 attitude, e3 support and interpersonal relationships, e5 service system and policy, as well as attitudes and support of family members and colleagues, interpersonal relationships, social support, transportation, equipment and facilities, and policies. Conclusion:Rehabilitation exercise is an important part of health-related services. We scoped the recreational physical activities and its functional effects for adults with disabilities. The categories and structure of recreational physical activities and rehabilitation exercises were constructed based on ICF. Aligning rehabilitation exercises and relevant factors to ICF, the recreational activities and rehabilitation exercises had been reviewed and discussed and several recommendations for further studies had been drawn.

ABSTRACT

Introducción: la ruptura del ligamento cruzado anterior de la rodilla es una de las lesiones que se presenta frecuentemente en las personas que realizan deporte o actividad física. Objetivo: comparar los tratamientos funcionales utilizados para la recuperación de la ruptura de ligamento cruzado anterior. Metodología: se realizó una revisión bibliográfica de artículos científicos publicados desde enero del 2000 hasta enero de 2018. Las bases de datos consultadas fueron Medline, Scopus y Scielo. Utilizando las siguientes palabras claves: lesión de rodilla, ligamento cruzado anterior, rehabilitación funcional. Se seleccionaron aquellos estudios en los que se hubiese analizado lesiones de ligamento cruzado anterior en cualquier disciplina deportiva. Resultados: se encontró seis estudios, todos de aplicación en los diferentes niveles de experimentación (pre-, cuasi- y experimental). Discusión y conclusiones: el ejercicio funcional, prescrito de una manera individualizada, con un seguimiento mediante test apropiados que se realicen antes durante y después de la rehabilitación de lesiones del ligamento cruzado anterior reduce los tiempos de recuperación de la lesión.

OBJECTIVE@#To compare the effect of floating needle therapy on myofascial trigger point (MTrP) and conventional electroacupuncture on the recovery of shoulder joint function after surgical neck fracture of humerus on the basis of drugs and rehabilitation training.@*METHODS@#A total of 72 patients with unilateral surgical neck fracture of humerus were randomly divided into a floating needle group (35 cases) and an electroacupuncture group (37 cases). At the same time of the basic treatment, both groups were intervened on the 2nd day after operation. Touched the MTrP at the affected limb, and the floating needle was used to sweep around it in the floating needle group. In the electroacupuncture group, electroacupuncture was applied at Hegu (LI 4), Quchi (LI 11), Waiguan (TE 5) on the affected side, the dilatational wave (4 Hz/20 Hz) was selected. The treatment was given once every day for 2 weeks. Pain visual analogue scales (VAS) score, constant shoulder joint score, and changes in the number of MTrP were used to evaluate the pain and functional status of the shoulder joints before, after treatment, and 3 months after treatment.@*RESULTS@#Compared with that before treatment, the VAS scores were decreased, the constant scores were increased, and the numbers of MTrP were decreased in the two groups after treatment and 3 months after treatment (<0.05), but the improvement in the floating needle group was better than that in the electroacupuncture group (<0.05). Compared with that after treatment, the VAS scores were decreased and the constant scores were increased 3 months after treatment in the floating needle group (<0.05), and the VAS scores in the electroacupuncture group were decreased (<0.05).@*CONCLUSION@#Floating needle therapy is helpful for the rehabilitation of shoulder joint function after surgical neck fracture of the humerus, and its curative effect is better than conventional electroacupuncture.

ABSTRACT

There is no consensus on the best treatment for acute Achilles tendon rupture. Functional rehabilitation after surgical treatment has been widely recognized, but the functional rehabilitation of non-surgical treatment has not been specifically studied. Based on the clinical practice of all kinds of Achilles tendon rupture, the best dynamic functional rehabilitation scheme and evaluation method were discussed. In order to bring more effective rehabilitation strategies for patients with Achilles tendon rupture, and reduce the rate of re-rupture and complications.

ABSTRACT

Objective: To summarize the latest research progress of early postoperative rehabilitation for acute Achilles tendon rupture after surgical repair. Methods: The high-quality randomized controlled trials and systematic reviews/meta-analyses regarding early postoperative rehabilitation for acute Achilles tendon rupture in recent years were reviewed. Results: There are three functional rehabilitations after acute Achilles tendon rupture surgery, including early postoperative mobilization, early weight-bearing with immobilization, and early weight-bearing combined with mobilization. The results of randomized controlled trials show that the effectiveness of early postoperative rehabilitation is similar or better than the early postoperative immobilization. The results of systematic reviews/meta-analyses show that the early postoperative rehabilitation is beneficial to the early function recovery of the Achilles tendon, can reduce the time for functional recovery, and do not adversely affect the outcomes. Conclusion: Early postoperative rehabilitation is beneficial to the functional recovery, and do not increase postoperative complications. There is still no uniform protocol of early postoperative rehabilitation and the timing of weight-bearing, and further studies are needed in the future.

ABSTRACT

Abstract Introduction The expansion of assistive technologies has been increasing significantly, involving almost all the daily activities, performed by disabled people. One of the highlights to be mentioned is the robotic orthoses, which are being used to amplify movements, to replace amputated limbs, and for a variety of rehabilitation therapies, among others. This study aims to present a review about upper limbs' orthoses, discussing their weaknesses, potentialities, and pointing out for future perspectives under the motor rehabilitation. Methods For this research, it had been identified 161 papers, based on the bibliographic bases from IEEE and Science Direct, which were filtered by the keywords orthoses, hand, upper-limbs and technology. After the appropriate exclusions, 22 papers were analyzed. Results This review indicates that most of the orthoses have been developed for rehabilitation therapies to be employed by people who have suffered stroke, cerebral palsy or spinal cord injury. This research summarized that the upper limbs movements are effectively produced by electric or pneumatic actuators, having a variety of degrees of freedom. Conclusion This review has shown that, although there are many orthoses styles, ranging from electric to pneumatic actuators; also there are some technical restrictions that prevent their use by most people. However, upper limb orthoses seem to be a great solution for the rehabilitation of people who have suffered a stroke.

Objective@#To explore the effect of active vitamin D supplements on promoting postoperative lumbar functional rehabilitation in postmenopausal women.@*Methods@#From January 2015 to January 2017, selecting 120 postmenopausal women patients who underwent lumbar posterior surgery with age from 50 to 80 years, randomly divided into control group and observation group, control group and observation group suffer traditional nursing and treatment, meanwhile, observation group was supplied with active vitamin D, over a follow-up period of six months, using the Visual Analogue Scale(VAS)score, Japanese Orthopedic Association Scores(JOA)score, back stretch height to assess the effect of active vitamin D supplements.@*Results@#Back stretch height of patients in the observation group was (25.4 ± 2.6) cm, which was significantly better than (20.7 ± 2.1) cm of the control group after 6 months (t=-10.90, P<0.01); the observation group JOA score and VAS score were significantly better than the control group (JOA score: 25.8±2.0 vs. 24.6±1.8, t=-3.50, P<0.01; VAS score: 1.6±0.9 vs. 2.1±1.1, t=-3.10, P<0.01).@*Conclusion@#Active vitamin D supplements can improve the patient′s waist discomfort, enhance the muscle strength, improve the quality of life, and get better functional rehabilitation.

Objective To observe the effect of thumb pressing at Yangming Meridian on the function rehabilitation after total knee arthroplasty (TKA) for knee osteoarthritis (KOA).Method Sixty KOA patients after TKA were randomized into a treatment group (thumb pressing at Yangming Meridian plus conventional rehabilitation) and a control group (single conventional rehabilitation),30 cases each.The treatment group was intervened by thumb pressing and kneading manipulation at the acupoints of Foot Yangming Meridian and Foot Taiyin Meridian plus rehabilitation training of passive flexion and extension by CPM machine,while the control group was intervened by rehabilitation training of passive flexion and extension by CPM machine.The American Hospital for Special Surgery (HSS) for knee joint,the American Knee Society score (KSS) and the Western Ontario and the McMaster Universities Osteoarthritis Index (WOMAC) of KOA were used to evaluate the function.Result After the treatment,the HSS,KSS and WOMAC scores were changed significantly in both groups (P<0.05).The improvements of HSS,KSS and WOMAC scores in the treatment group were superior to those in the control group,and the differences were statistically significant (P<0.05).Conclusion The efficacy of thumb pressing at Yangming Meridian plus conventional rehabilitation for post-TKA KOA is superior to single conventional rehabilitation.

ABSTRACT

La Terapia Ocupacional es una disciplina cuyo marco conceptual, objetivos y métodos están bien establecidos. Uno de los ámbitos en los que interviene es el de la adicción. Sin embargo, los textos y artículos ocupacionales disponibles carecen, en muchos casos, de un marco explicativo propio, recurriendo con demasiada frecuencia a otros importados de disciplinas preponderantes. Ello lleva, por una parte, a una pobre producción científica y, por otra, a un rol secundario de nuestra disciplina en los programas terapéuticos. El presente trabajo recoge la propuesta de un marco explicativo de la adicción desde una perspectiva ocupacional. Algunos autores parten de considerar que la propia adicción es, en sí misma, una ocupación, en la medida en que proporciona un rol y una significación al propio adicto. Desde esta perspectiva, toman sentido ocupacional paradigmas experimentales recientes, como el del enriquecimiento ambiental, o conceptos como el de empoderamiento. La rehabilitación cognitiva funcional, ampliamente aplicada en clínica del daño cerebral, representa otro enfoque complementario en la clínica de la adicción. Existen instrumentos de evaluación propios que cuantifican o sistematizan las variables ocupacionales relevantes. La Terapia Ocupacional está en condiciones de asumir un rol propio, protagonista e irrenunciable en el tratamiento de la adicción.


Occupational Therapy is a discipline whose conceptual framework, objectives and methods are well established. Addiction is one of the areas in that is involved. However, the available occupational texts lacking in many cases its own explanatory framework, resorting too often to other imported from prevailing disciplines. This leads, on the one hand, to a poor scientific production and, on the other one, to a secondary role for our discipline in therapeutic programs. This paper presents a proposed explanatory framework of addiction from an occupational perspective. Some authors start considering that addiction is itself an occupation, to the extent that it provides a role and significance to the addict himself. From this perspective, it makes occupational sense some recent experimental paradigms, such as environmental enrichment, or concepts such as empowerment. The functional cognitive rehabilitation, widely applied in clinical of brain injury, represents another complementary approach to addiction clinic. There are own assessment instruments that measure or systematize the relevant occupational variables. Occupational Therapy is able to take its own role, main and indispensable prominence in the treatment of addiction.

Resumen El progreso de la bioingeniería ha permitido el desarrollo de herramientas tecnológicas para cuantificar las variables asociadas al examen físico. En este artículo se presenta la descripción, ventajas y limitaciones de tecnologías para la medición de fuerza, arcos de movimiento, equilibrio y estabilidad, coordinación y marcha. Se proponen también alternativas para uso de estas tecnologías como parte del proceso de valoración de pacientes complejos, ya sea porque se sospeche simulación o porque presenten lesiones graves en las que se requiera sugerir opciones en el proceso de rehabilitación o cierre de caso. En tales circunstancias se identifican alternativas para un diagnóstico más completo que disminuyan la subjetividad del terapeuta y con mayores posibilidades de monitoreo.


Abstract The progress of bioengineering has allowed the development of technological tools to quantify the variables associated to the physical assessment process. The description, advantages and limitations of technologies to measure the strength, range of motion, balance and stability, coordination and gait are shown. Some alternatives are proposed for using those technologies as part of the appreciation process in complex patients, either for any suspicion of simulation or because any serious injury can exist where some alternatives for rehabilitation process are required. In such cases, must be identified some options for a more complete diagnosis, decreasing the subjectivity of the therapist and with more monitoring possibilities.

Objective To explore the effect of early rehabilitative intervention on postoperative functional rehabilitation of artificial femoral head replacement,to provide the basis for clinical practice.Methods Sixtyfour patients received artificial femoral head replacement were divided randomly into the rehabilitative group and the control group with 32 patients in each group.The patients in the control group received routine rehabilitation exercises,while the rehabilitative group received rehabilitative intervention after 3 hours of operation.The incidence of complications was compared after 2 weeks,and Harris scores were compared after 6 months.Results The incidence of complications in the rehabilitative group was significantly lower than the control group.Harris scores of the rehabilitative group were significantly better than the control group.Conclusions Early rehabilitative intervention can decrease the incidence of complication and accelerate the functional recovery of affected limbs after artificial head replacement.

ABSTRACT

Objective To observe the effect of sensory reeducation on the motor function and activities of daily living (ADL) in stroke patients with proprioceptive disturbance. Methods Sixty patients with proprioceptive disturbance after stroke were randomly divided into the treatment group and observation group, and 30 stroke patients with no proprioception disturbance were recruited as the controls. Three groups were treated with routine rehabilitation interventions, but the treatment group were given sensory reeducation in addition to the regular treatment. Fugl-Meyer motor assessment scale (FMA) and sensory assessment scale, and with Barthel index (MBI) for activities of daily living (ADL) were used to evaluate the patients before and after treatment. Results FMA and MBI scores of patients in three groups were increased significantly (P<0.01) after treatment. The scores of patients in observation group were significantly lower than those of the control group and treatment group (P<0.01). Those of the treatment group and the control group were with no significant difference (P>0.05). Conclusion The proprioception disturbance significantly affects the rehabilitation process, and sensory reeducation can improve the sensory function, motor function and activities of daily living (ADL).

Objective:To evaluate the clinical value of determinations of C-reactive protien concentration in serum and cerebrospinal fluid from patients with acute and recovery cerebral infarction and to find out the correlation of it with neurologic deficits,infarct sige and duration.Methods:46 cases of patients with cerebral infarction were recruited as observe group and 25 cases of healthy examinees or patients undergoing operation of fracture,appendix or cholecyst ectomy were recruited as control group.We measured C-reactive protien in cerebrospinal fluid and serum.Comparison analysis was performed using a SPSS10.0 statistical package.Results:Concentration of C-reactive protien in cerebrospinal fluid and serum was significantly higher in patients with cerebral infarction than that in patients at recovery stage and control group(P
